feat(执行观察器): 重构 Planner 轴 动态任务树 与任务检查器

主界面改为真实运行选择、Planner 行为时间轴、随规划生长的 Task 树和右侧渐进式详情。支持生命周期、Worker Loop、数据依据、代码/Prompt 标签,并区分焦点、变化、验收和阶段边界状态。
